This service is intended only for non-emergency health concerns that may be addressed through telehealth.

If you believe you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency room.

Telehealth is not a substitute for in-person care in all situations.

Arbitration and class action waiver notice

These Terms include a dispute resolution and arbitration section that affects your rights.

In most cases, disputes must be resolved by individual arbitration and not as a class action.

These Terms

These Terms and Conditions (Terms) govern your access to and use of this website and any pages where these Terms are posted (Website), plus any related online tools, portals, forms, messaging, scheduling, and payment features made available through the Website (Platform).

The Platform and certain administrative support functions may be provided using technology and operational support services from a third-party telehealth operations provider. If you access the Platform through a co-branded experience, embedded page, or link from another site, these Terms still apply.

By clicking “I Agree,” “Continue,” “Proceed,” “Register,” “Sign Up,” or a similar button, or by otherwise accessing or using the Website or Platform, you agree to these Terms and any policies referenced in these Terms.

If you do not agree, do not use the Website or Platform.

Definitions

Company, we, us, or our means Coach T Medical Devices LLC.
You or your means the individual accessing or using the Website or Platform.
Clinician means an independent, licensed healthcare professional and or clinician group who may provide clinical services to you.
Third Parties means entities other than us, including Clinicians, pharmacies, laboratories, shipping carriers, payment processors, and technology providers.
Content means text, graphics, images, audio, video, documents, downloads, trademarks, logos, designs, software, and other materials made available through the Website or Platform.
User Content means information, text, images, videos, messages, documents, and other materials you submit through the Website or Platform.

Website content and informational use

The Website may include general information about services, telehealth concepts, wellness topics, conditions, or products. This information is provided for general informational purposes only.

We do not provide medical advice through the Website. Do not rely on Website content as a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified healthcare professional with questions you may have regarding a medical condition.

What we do and what we do not do

We provide technology and administrative services that help users access telehealth-enabled care and coordinate related non-clinical functions. This can include account creation, intake collection, scheduling support, customer support, payment processing, and communications tools.

We are not a medical group and we do not practice medicine. We do not provide diagnoses, treatment, or prescriptions.

Clinical services, if offered through the Platform, are provided by independent Clinicians. Clinicians are solely responsible for clinical decision-making, including whether telehealth is appropriate, what care to provide, and whether to prescribe medication.

Platform support and support contacts

The Platform may be powered, supported, and administered in whole or in part using technology and operational support services from Healthaide LLC, including for technical support, intake collection, scheduling support, communications tools, and payment processing.

Coach T Medical Devices LLC provides general and non-clinical support for users, including help with account access, basic navigation, general questions about programs or services, and customer service issues that are not medical in nature.

Healthaide LLC provides medical support exclusively. Medical support means facilitating access to clinical services and helping route clinical questions through the Platform to independent licensed clinicians.

Healthaide LLC is not the patient’s treating clinician. Clinical services, if offered through the Platform, are provided by independent licensed clinicians and or clinician groups (Clinicians). Clinicians are solely responsible for clinical decision-making.

If you have a medical question, you should direct it to your Clinician through the Platform. If you are unsure how to contact your Clinician, contact support@healthaide.io for assistance routing your request.

Separate telehealth consent

To receive telehealth services, you may be required to review and accept a separate telehealth consent. That consent is incorporated into these Terms by reference.

Changes to the Website, Platform, and these Terms

We may update the Website, Platform, Content, and features from time to time, and we may suspend or discontinue any part at any time.

We may update these Terms from time to time. Changes are effective when posted. Continued use means you accept the revised Terms.

Availability, eligibility, and verification

Services are not available in all states or jurisdictions and may change.

We may request proof of identity and location and may decline service if eligibility cannot be verified.

Account registration and security

You agree to provide accurate and complete information and keep credentials secure.

You are responsible for all activity under your account and must notify us of unauthorized access.

Electronic communications and e-sign consent

You consent to receive communications electronically, including email, phone, SMS, or in-app messaging.

Message and data rates may apply. You may opt out of SMS by replying STOP, though this may limit communications.

Electronic signatures have the same legal effect as physical signatures.

Not for emergencies

The Platform is not monitored for emergencies.

Call 911 if needed.

Your responsibilities and accurate information

You must provide accurate and complete information and follow Clinician instructions.

Failure to do so may result in harm, delays, or denial of service, and you agree not to hold us responsible for such outcomes.

Telehealth limitations and clinical appropriateness

Telehealth may not always be appropriate. Clinicians may require additional steps or direct you to in-person care.

We do not guarantee prescriptions, outcomes, or availability.

Prescriptions, pharmacies, and fulfillment

Prescriptions may be sent to pharmacies. Pharmacies are independent Third Parties.

You may remain responsible for fees once services or prescriptions are initiated.

Laboratory testing and diagnostics

Labs are independent Third Parties. Results may be imperfect or delayed.

Clinicians interpret results and may recommend follow-up.

Products, shipping, and special handling

Shipping is not guaranteed. You must provide accurate details and follow product instructions.

Risk of loss may transfer upon delivery.

Fees, billing, and payment authorization

You agree to pay all fees shown at checkout.

You authorize charges to your payment method, including recurring charges if applicable.

Subscriptions and automatic renewals

Subscriptions will renew unless canceled.

Canceling stops future billing but does not refund current charges.

Cancellations

Cancellation policies are disclosed at checkout.

You may still owe fees if services have already started.

Refunds, returns, and payment disputes

All sales are final unless required by law or approved by us.

Non-refundable items generally include:

  • clinical services already rendered
  • administrative fees
  • prescriptions once processed
  • shipped or opened products

Chargebacks without contacting support may result in account suspension.

Privacy, health information, and security

Our Privacy Policy is available at sygnlco.com.

We use reasonable safeguards but cannot guarantee complete security.

User Content, feedback, and permissions

You grant us a license to use submitted content to operate the Platform.

Feedback may be used without restriction.

Acceptable use

You agree not to misuse the Platform.

Prohibited actions include:

  • false information
  • impersonation
  • unauthorized access
  • malware
  • harassment

Violations may result in termination.

Intellectual property and trademarks

All content is owned or licensed by us.

You may not copy or misuse it without permission.

Third-party services and links

We are not responsible for Third Party services or policies.

Disclaimers

Services are provided as is and as available.

We disclaim all warranties to the fullest extent permitted by law.

Limitation of liability

We are not liable for indirect or consequential damages.

Our total liability is limited to the amount you paid for the service.

Assumption of risk

You accept the risks of telehealth and electronic communication.

Indemnification

You agree to indemnify us against claims arising from your use or violations.

Termination

We may suspend or terminate access at any time.

You remain responsible for outstanding payments.

Governing law

These Terms are governed by the laws of the State of Texas.

Dispute resolution and arbitration

Disputes must first be attempted informally.

If unresolved, disputes will be handled via binding individual arbitration in Texas.

You waive the right to a jury trial.

You may opt out within 30 days by emailing hello@sygnlco.com.

Time limit to bring claims

Claims must be brought within 1 year.

Force majeure

We are not responsible for events beyond our control.

Assignment

We may assign these Terms; you may not without consent.

Severability

Invalid provisions do not affect the rest.

No waiver

Failure to enforce does not waive rights.

Survival

Certain sections survive termination.

We may notify you via website, app, or email.

Entire agreement

These Terms constitute the full agreement between you and us.
